Kabayan - We had finally received a formal announcement from the Philippine Embassy in Ottawa about Consular Outreach Mission service in New Brunswick.  Appointment slots will be given out on a first come, first served basis so call immediately to reserved your spot. You might be able to negotiate the preferred day that you can come to the venue because of your work/personal situation.


Here are more detailed information below:

DATE/TIME: Friday, 14 October, 6:00-9:00 PM
                   Saturday, 15 October, 9:00 AM - 8:00 PM
                   Sunday, 16 October, 9:00 AM - 12:Noon
VENUE:    Moncton Lions Senior Citizens Club (click here for the map)
               473 George Street, Moncton, NB E1C 1Y2
            
        Applicants are requested to take note of the following requirements:

1) Application for passport renewal

To ensure an orderly delivery service, interested kababayans are requested to contact Ms.Merly Halili or Ms. Jiji Gonato of the Philippine Embassy in Ottawa (Tel: 1 (613) 233-1121) immediately for an appointment slot. Appointment slots will be given out on a first come, first served basis. Accepted applicants are requested to:

          - Come to the venue on the appointed date/time in decent attire with collar and sleeves (avoid wearing shirt/blouse in light blue color) for the photograph taking. They will also be asked to remove earrings, eyeglasses, hair accessories, and any facial jewlery.
 
          - Bring the following:
             a) duly-accomplished e-passport application form (downloadable from http://philembassy-ottawa.com/home/images/stories/EPASSPORT_APPLICATION_FORM_updated_30_Jun_2011.pdf)
             b) original passport
             c) photocopy of date page (or first page) of passport
             d) photocopy of work permit or permanent resident card
             e) Cash payment of $69 CAD (exact amount only)
             f) Xpresspost envelope with SIGNATURE STICKER (Please fill out complete return address with your telephone numbers).

2) Application for notarial services
             a) Download and present the duly-accomplished document to be notarized (prescribed forms may be downloaded from www.philembassy-ottawa.com)
             b) Photocopy of date page (or first page) of passport
             c) Photocopy of work permit or permanent resident card
             d) Cash payment of $28.75 CAD per document (exact amount only)
             e) Xpresspost envelope WITH SIGNATURE STICKER (Please fill out complete return address together with your telephone numbers)

3) Application for dual citizen ship
             a. Duly-accomplished application form (downloaded from http://philembassy-ottawa.com/home/images/stories/DUAL_CITIZENSHIP_pdf.pdf)
             b. Original and photocopy of Philippine birth certificate (original document will be returned to applicant)
             c. Three (3) recent 4.5cm x 3.5cm photographs with royal blue background;
             d. Canadian naturalization document or citizenship card
             e. Canadian passport
             f. Processing fee of CAD $ 57.50

4) NBI clearance
             a) Complete NBI Form No. 5 (available form consular officer)
             b) One 2"x 2" photograph
             c) Cash payment of $28.75 CAD (exact amount)

Kabayan,

   Early this year, we had discussed a CommUNITY initiative to host a consular outreach service in New Brunswick particularly in Fredericton. Based on the information that we had collected so far, Fredericton is a central proximity to majority of Filipinos needing this service.

   So on May 2nd, I had sent a request letter to Consul Porfirio Mayo (Philippine Embassy in Ottawa) to have an electronic passport mobile service in Fredericton. Moreover, we made an offer to partner with the embassy to sponsor and facilitate this event. From that time, I have been in touch with him several times to have this service in July or August. However they could not make the decision and provide more information up to this time. Consul Mayo, however said that they will have a definite information by 2nd week of August for a September consular outreach service.

  Once I hear from him, I will let you know so you can start the renewal process. Please share this information other Filipinos that might benefit from this service and have them contact us.

   I have been receiving an inquiry from our kababayans regarding a need for Philippine passport renewal this year. I had inquired with the embassy in Ottawa to have a remote consular outreach in Fredericton tentatively in August.

Let me know if you would like to avail this service. Since you are my contact in your area, please forward this email to other kababayans in your city/community who need this service. The embassy required a substantial number of renewal to have this approve. They also have a maximum number of renewal, so let me know if you are interested to take advantage of this initiative. This will tremendously save you lot on travel cost, accomodation and time. They can provide the following services:
    - E-passport renewal
    - Application for Dual Citizenship
    - Application for Notarial services
    - NBI Clearance
