Applicants are requested to take note of the following requirements:
1) Application for passport renewal
To ensure an orderly delivery service, interested applicants should contact Ms.Merly Halili or Ms. Jiji Gonato of the Philippine Embassy in Ottawa (Tel: 1 (613) 233-1121) for an appointment slot. Appointment slots will be given out on a first come, first served basis.
Accepted applicants are requested to:
- Come in a decent attire with collar and sleeves (avoid wearing shirt/blouse in light blue color) for the photograph taking. They will also be asked to remove earrings, eyeglasses, hair accessories, and any facial jewelry. - Bring the following: a) duly-accomplished e-passport application form (downloadable from http://philembassy-ottawa.com/home/images/stories/EPASSPORT_APPLICATION_FORM_updated_30_Jun_2011.pdf) b) original passport c) photocopy of date page (or first page) of passport d) photocopy of work permit or permanent resident card e) Cash payment of $69 CAD (exact amount only) f) Xpresspost envelope with SIGNATURE STICKER (Please fill out complete return address with your telephone numbers).
2) Application for notarial services a) Download and present the duly-accomplished document to be notarized (downloaded from www.philembassy-ottawa.com) b) Photocopy of date page (or first page) of passport c) Photocopy of work permit or permanent resident card d) Cash payment of $28.75 CAD per document (exact amount only)
3) Application for dual citizen ship a. Please see list of requirement from www.philembassy-ottawa.com
4) NBI clearance a) Complete NBI Form No. 5 (available form consular officer) b) One 2"x 2" photograph c) Cash payment of $28.75 CAD
